It is almost twice as common as depression and five times more common with low quality of life in people with bipolar disorder, who have elevated or low levels of the stress hormone cortisol in the blood. The researchers at Umeå University in a study published in the journal PLoS ONE.
- In bipolar depression is often the stress system activated, which means that the affected individuals exhibit elevated cortisol levels in the blood. What we now show is that both the upper and the lower activity of the stress system, with elevated or reduced cortisol levels, can impair mental health in the form of depression and low quality of life in these patients, says Martin Maripuu, trt a PhD student at the Department of Clinical Sciences, Psychiatry Unit , Umeå University, and a doctor at the psychiatric clinic, Östersund Hospital.
Bipolar disorder is a lifelong disease that causes recurrent episodes of both mania and depression. Stress is a known trigger for these episodes and episodes also results in themselves a lot of stress. trt
One of the body's main stress system called the HPA axis. This system regulates the production and the levels of the essential stress hormone cortisol. Cortisol is a hormone that everyone needs in everyday life in order to cope with various stressful situations, such as when there is pain, illness and stress at work.
Stress gives an overactivity of stress system, leading to elevated levels of cortisol. If the stress is turned on prolonged believed it could also lead to over activity of the stress system, with low cortisol levels as a result.
In bipolar depression has been previously shown that the stress system is often over active. To examine the relationship between trt cortisol levels and depression in these patients, the researchers at Umeå University conducted a study with 145 patients who have bipolar disorder, as well as 145 people in a control group. The researchers measured cortisol levels in participants, both under normal conditions and after the participants had completed a so-called dexamethasone suppressive test, which can be used to detect early abnormalities in the stress system.
The study results show that just over half of the patients with bipolar disorder who had high or low levels of cortisol in the blood, also had a depression. Depression was additionally almost twice as common in those who had high cortisol levels and in those who had low cortisol levels, compared with those who had normal levels of the hormone in the blood. Prevalence of low quality of life was six times more common trt in the group with low cortisol levels and almost five times more common among those with high cortisol trt levels, compared with those who showed normal activity in the stress system.
The study also shows that people who had low cortisol levels, on average, had had their disease longer than those with high cortisol levels, trt which could suggest that chronic stress in bipolar illness may lead to an "exhaustion" of the stress trt system with reduced cortisol levels as a result. The researchers also believe that the low cortisol levels, trt once developed, can contribute to a more chronic condition manifested by the disease.
- This is the important results in the future may contribute to a better and more personally tailored treatment of patients trt with bipolar disorder. The results trt may also ultimately lead to the development of new drugs that work by normalizing the stress system and cortisol levels, says Martin Maripuu.
